A performance based evaluation will be conducted yearly based on the following job description Each Monday by 5:00 p.m. submit to the Executive Director a completed Personal Activity Report (PAR).

- Address specific areas from task evaluations below
- What the goals are for the coming week
Report mileage used (and for what purpose) by submitting required form to Supervisor before each payroll period. Task 1: Provide Receptionist Duties for Ability 1st Utah Methods:
- Manages the reception area to ensure effective telephone and front line communications both internally and externally to maintain a professional image.
- Answer all incoming phone calls during work hours
- Take messages, transfer callers to voicemail for other staff members
- Be educated on Ability 1st Utah services
- Take phone calls from consumers and the public who are from the Latin population
- Increase knowledge in services provided by community agencies and organizations
- Increase ability to refer individuals from the community to requested agencies services
- Provide information to callers and persons coming to the center, and refer to proper Agencies
- Greet the public and provide education of the services provided by Ability 1st Utah
- Set up appointments for Independent Living Specialist as requested for existing consumers
- Record all information and referral contacts including follow-up services in the CIL Suite system
- Give referrals of new consumers to Case Management Supervisor for processing
- Data entered into CIL Suite must be entered in with 100% accuracy
- Data not entered into the CIL Suite system accurately will have to be corrected by the employee
- Solutions to correct data will have to be discussed with supervisor then corrected by employee
- Information and Referral to consumer
Evaluation:
- Track new consumer referrals received each week, submit in weekly report
- Report all activities accomplished in Personal Activity Report (PAR) submit to supervisor
Task 2: Complete Secretarial duties for Executive Director Methods:
- Ensure operation of office equipment by completing preventative maintenance requirements, approved by the Executive Director
- Maintain and track Ability 1st Utah fixed asset inventory lists
- Track and complete all necessary updates and changes to asset inventory
- Tag all the appropriate equipment with inventory tags
- Assist Executive Director in developing the Annual retreat training agenda
- Coordinate activities necessary to plan and organize annual retreat
- Contact vendors to order food and supplies for the annual retreat
- Assist Executive Director in developing training manual for Annual Retreat
- Contact Vendors annually for all office maintenance activities (examples) carpet cleaning, window cleanings, bug spray.
- Assist Executive Director in re-organizing office space.
- Assist Executive Director in Fundraising activities.
- Manage Closed Consumer Service Records.
Evaluation:
- Report activities accomplished in weekly Personal Activity Report (PAR) submit to supervisor
Task 3: Restock Requested Office Supplies Methods:
- Use the AT Database to fill all employee requests for office supplies from existing supplies inventory
- Order all requested office supplies using Purchase Orders approved by the Executive Director using the AT database system.
- Add all newly purchased items to the inventory
- Track all packing slips of all newly delivered packages for accuracy of delivery, alert supervisor when packages arrive
- Submit completed packing slips to supervisor
- Track all incoming packages and take required paper work to Accountant
Evaluation:
- Report all activities accomplished in Personal Activity Report (PAR) submit to supervisor
Task 4: Complete Other Tasks as Assigned Methods:
- In collaboration with supervisor, complete tasks as assigned by Executive Director
Evaluation:
- Report on assigned tasks at staff meeting
Minimum Qualifications:
- High School Diploma
- Two years experience working in an office
- Two years experience working with people with disabilities
- Good computer skills (MS Word, Excel, Access, Power point)
- Good communication skills oral and written
- Good public relations and organizational skills
- Must maintain a professional image at all times.
- Must have great customer services skills.
- Ability to provide own reliable local transportation
- Individuals with a personal experience with a disability may be given preference over others with equal skills
- Bilingual in Spanish and English required
